Problems with your The Ohio SD 100 (School District Income Tax) may have happened due to a data mismatch between your W-2 entries and the State interview section. Below are three (3) possible fixes that you can try.
1. Enter/Edit/Delete Forms Online:
Open or continue your return.
Click on Tax Tools, then Tools (bottom left).
Click on Delete a Form.
Find and Delete Form SD 100 (and any "School District" worksheets) in the list. Also look for any "Untitled" or "Duplicate" Ohio forms and delete those as well.
Click on the State tab and re-run the Ohio interview.
2. Enter/Edit/Delete W-2(s):
Open or close your return.
Under your Federal tab, Click on Wages & Income, and then Click on W-2(s).
Box 20 must contain the 4-digit school district code (e.g., 2509). Make sure that there is not a name (like "Columbus") instead of a code, or if the code is correct.
3. Perhaps you may need to clear your cache and cookies, make sure that your W2 is indeed deleted, and then re-enter your W2 manually.
Clear your cache
Your browser stores recently visited webpages as temporary files in a cache, which enables the pages to load more quickly when revisited.
However, a full or corrupted cache can cause problems in TurboTax, so sometimes you need to clear your cache (that is, remove these temporary files).

Apple's official support site doesn't appear to contain instructions for clearing the cache in Safari. Your best bet is to search the Internet for clear Safari cache or similar search terms.
Clear your Cookies
Cookies are small files that temporarily store data on your computer. We use them to personalize your experience so you see information you already entered and not items that are irrelevant or repetitive.
On rare occasions, they may make your browser think you already did something that you still need to do. To fix that, you'll need to delete (clear) the cookies from your browser.
