It seems that the IRS itself has some sort of glitch that is triggering these notices to go out unnecessarily.    You should be safe to ignore it and just get your return filed by the October 15 deadline.   We have been seeing some other posts from users who received these notices.

Sorry--no there is nothing to enter on your tax return about that.   Only if you have been diagnosed as "legally blind" --then you get an extra amount on your standard deduction Higher Standard Deduction for Blindness   If you are blind on the last day of the year and you don't itemize deductions, you are entitled to a higher standard deduction.   Not totally blind.    If you aren't totally blind, you must get a certified statement from an eye doctor (ophthalmologist or optometrist) that: 1You can't see better than 20/200 in the better eye with glasses or contact lenses, or 2Your field of vision is 20 degrees or less.      If your eye condition isn't likely to improve beyond these limits, the statement should include this fact. Keep the statement in your records.     If your vision can be corrected beyond these limits only by contact lenses that you can wear only briefly because of pain, infection, or ulcers, you can take the higher standard deduction for blindness if you otherwise qualify.

It would only be taxable if the refund was more than the premiums you paid, and then only the amount over would be taxable.

In Step 3, to e-file your tax return, you must click on the large button labeled "Transmit my returns now".   After completing the File section and e-filing your tax return you will receive two emails from TurboTax. The first email when your tax return was transmitted and the second email when the tax return has either been accepted or rejected.   Note - Once a tax return has been Accepted by the IRS or a State, TurboTax receives no further information concerning the tax return or the status of any tax refund.  Only the taxpayer listed on the tax return can obtain the status of a tax refund or a tax return.   To check the status of an e-filed return, open up your desktop product or log into your TurboTax Online Account. You can find your status within the TurboTax product. If accepted by the IRS use the federal tax refund website to check the refund status - https://www.irs.gov/refunds If accepted by the state use this TurboTax support FAQ to check the state tax refund status - https://ttlc.intuit.com/turbotax-support/en-us/help-article/tax-refund/track-state-refund/L3jgO8PGs_...   After the tax return has been Accepted by the IRS (meaning only that they received the return) it will be in the Processing mode until the tax refund has been Approved and then an Issue Date will be available on the IRS website.

Clearly, there is a disconnect between your thought process and the reality of this situation.   Moreover, there is no "3500% payoff" if either (a) the taxpayer here never does use some sort of "Medicaid program" to pay for long-term nursing care (beyond what is covered by Medicare) OR (b) the taxpayer executes a QC deed in short order, which would appear to have little to ZERO adverse consequences in this case.      @Opus 17 wrote: 2. It depends.   If the paid family leave was paid by your employer on your W-2, then it would be taxable on both your federal and state return.  If it was paid by the EDD and reported on Form 1099-G or paid by VPDI, then it would not be taxable on your state return.  See this link for more details.     If you determine your return is in error and you need to amend your returns, please see this link for more guidance.

A transcript is something you can get from the IRS that shows information from your tax return, but it is not a copy of your tax return. If you e-filed you didn't sign the tax return when you sent it to the IRS, so there is no way to get a signed copy from TurboTax or from the IRS. If you need a signed copy of your tax return, do what xmasbaby0 said: print it and sign the printed copy.
