Or----did that total amount include a federal refund and a state refund?   Federal and state refunds do not arrive together; they are issued separately by completely separate entities.

If you are filing an amended return, the amount in your summary is likely your original amount you were getting back.  When you added additional income, you increased your tax liability, so you likely received more back than you should have.  When this happens, the amount due also includes paying back what you originally received that you shouldn't have received.   The amount on your form 1040 will still match the refund you saw in the summary, but then the other amount is likely what you will see on the 1040X.

To be specific, I only owe an additional $7, as a result of an increased Dividend amount.  I know only the 1040-X is needed along with corrected/changed forms, but since the corrected 1099 amount is only referenced on the first 3 pages, do I need to send in all 10 pages of that 1099?  Otherwise, I should be good.

Are you claimed as a dependent or not?   If you are claimed as a dependent on their return then you need to mark in the personal info section that you are claimed as a dependent and then you do not enter anything at all on your return about the insurance. If you are NOT claimed as a dependent on their return, then yes you can enter 0% since they claimed 100%.   If you have any issue with rejection of your return due to them saying they did not share the policy, then you would need to print and mail your return.
Do I need to send a copy of my 1099Rs if I e-filed and my return was accepted by the IRS?
Sorry. I attempted to efile the amended return multiple times and received same error message that this return has already been efiled

Not sure what you are trying to do.  You need to enter your W-2 exactly as it appears in the federal return, including the amounts from all of the boxes on the W-2.  Information flows from the federal return to the state return.   If you are entering information for a renter credit, you will be doing that when you prepare the IN tax return. You do not change anything on your W-2 in regard to a rent credit.
