enter the name on the 1099-R.

But the name on the 1099-R has the name of the Bank on the first line and the name of the company on the second line.  Which one is it - or is it both?

The TurboTax form 1099-R has two sections for names, enter both as shown on the 1099-R you received.

Just enter the first one and continue  since it doesn't really matter to the IRS ... only the EIN must be correct.
