My husband's company, of which he is the owner, hired our 15 year old daughter. Does she need to fill out a w-9 form?

The company is not an LLC, but incorporated.

No.  The purpose of a W-9 is to supply the employer with the employees SSN and  address so they can correctly report the employees income to the IRS. I am sure he already knows that without a W-9.

Technically it would be a W-4  not a W-9 but for the same reasons ... to get the employee's name, address and SS#  since a W-2 needs to be issued at the end of the year.   The child & company is not required to pay FICA taxes or even fed/state withholdings  but quarterly payroll tax forms do need to be timely filed.

I disagree.  The OP said it is incorporated.  That means that FICA applies.

You are correct ... if this is a corporation they still need to pay FICA taxes.

I mean it does depend on if the daughter is an employee or a contractor. I did not see this mentioned here at all. If they are an employee they need to fill out a W-4. If they are a contractor you would need a W-9. I mean you already have all the W-9 information, but it takes a few seconds to fill out.
