8962 form

I am trying to complete a 89-62 and a 1095A and I do not want to complete it wrong I just need a little assistance with the last four questions so that I can go ahead and send it back in and have my taxes completed

You do not complete the Form 8962.  The TurboTax program completes the 8962 after you have entered the Form 1095-A.
