Where do I enter the amount for Code DD (Cost of employer-sponsored health coverage)? Under "Any contributions you personally made (not through your employer)"?

Box 12 Code W and Code DD were entered under W-2 but only the Code W amount was auto-populated.

The Code DD amount doesn't go anywhere else on your return besides the W-2. For you it's FYI only; it is neither income nor a deduction.
