What needs to be included when submitting taxes by mail?

I have to file my taxes by mail this year because I've run to some problems with getting my AGI that matches the IRS database which is necessary to e-file. I've input the exact AGI from my return that I submitted last year, tried entering 0, and I've tried to request my transcripts on the IRS website but was unsuccessful.  I've tried creating an account for myself through the IRS website but they said my credit card info doesn't match what they have on file and then my wife tried but they said her phone number cant be linked to her name so therefore our identities cant be verified.  I have no choice but to file by mail at this point.  

 

I have a question about what exactly needs to be included when mailing my returns in, as this is my first time submitting by mail. What I've printed exactly for the federal return includes the instructions sheet, 1040 form,  "Additional Income and Adjustments to Income" Schedule 1, Schedule 2, Schedule C "Profit and Loss", Schedule SE "Self Employment Tax", form 8995, form 8962, and lastly an "electronic postmark-certification of electronic filing" (which says that my return was rejected by the IRS). 

 

I'm assuming that the instruction sheets and electronic postmark showing the rejection don't need to be included.  But, do all the other forms get mailed?

What needs to be included when submitting taxes by mail?

Yes, the other forms and schedules are included with the mailed federal tax return but Not the worksheets or the Postmark Report.  The address on where to mail is on the Federal Filing Instructions page.
