TurboTax Live Expert Missed IRA Rollover Form - Anyone Else Experience This?

I recently had an experience with a TurboTax Live expert who prepared my return without including my IRA rollover form. This oversight initially showed I owed over $3,000 in federal taxes! Only after I questioned this amount and specifically mentioned the missing IRA rollover did they correct it, bringing my actual tax owed down to $1,236.

Has anyone else experienced a similar situation with a tax expert missing important forms? I'm wondering if this is an isolated incident or more common. Also, has anyone successfully requested a refund or partial refund of service fees due to expert errors caught before filing?

Any advice would be appreciated. Thanks!
