One additional thing you could do is verify the correct the data format from your state tax authority. Merrill Lynch documentation may have an incorrect data format for your sate. For example, the State/Payer's State No in my Merrill Lynch statement included a forward dash [/] where there should not have been one. My state (Utah) treasurer website gave the correct format which I entered into Turbo Tax and it worked.
