Should I reply "both of us" to "This 1099-INT belongs to..."? The account belongs to both husband + wife but the INT form states that my wife's name is reported to IRS.

If you are filing Married Joint it really does not matter; however, if your wife's  name is listed on the 1099-INT then I would enter the information as listed on the 1099-R.

If you are filing Married Joint it really does not matter; however, if your wife's  name is listed on the 1099-INT then I would enter the information as listed on the 1099-R.
