Is there anything in Box 16 - State Tax Withheld?
Want to check that box number you're referring to? Box 17 is for wages and should contain a monetary amount. Box 16 is for the taxes withheld for that state, and the "other" box 16 is the state identifier. I would expect the "other" box 16 to be the one with ON in it.
If I"m right, then you will not report "ANY" of this income or taxes paid in the normal way you would enter a 1099-MSIC. That's because I'm of the impression this money was paid to you by a foreign business. If so, the "ON" is for taxes paid to Ontario and would be considered a foreign tax paid.
Does the business have a valid EIN in the "payer's TIN" box? It should be numeric and formatted XX-XXXXXXX
Is the Payer's address a valid U.S. address? (I suspect it's not)
