No, your estimated increase in Medicare Part B premiums cannot  be included as a "direct cost of selling" a piece of property.

 

Your Medicare premiums have no direct connection with your gain(loss) on the sale of land, nor would  anything else unrelated to the actual sale of the land. While your Medicare premiums are affected by your total income, any gain on the sale of land is only one factor in your total income.
