Form 1310 and Trust

Filing for a deceased person that had a trust, Do you check box C in part 1 ?  Then part 2- no, no, no , yes .  

Do you have to mail anything in with it?

Usually mailed with the final return. Check box C if no will then no, no, no, yes.
