Form 1099-INT from Department of the Treasury INternal Revenue Service has no street address, just Ogden, UT 84201. But Turbotax keeps asking me to fill in the street

put in any street. Maple street is pretty common.

Try putting Dept of the Treasury on the first line and Internal Revenue Service on the first line of the address

the only info required for a 1099-INT is payor and amount.  the rest is for the taxpayer's benefit and is not transmitted to the IRS when you e-file.
