I am using my dad as a co signer on my mortgage and they said I can provide something showing proof that he filed an extension (which his tax person did) along with his 2021 taxes and that would be fine. Is that something he gets from his tax specialist or the IRS directly?

Whoever filed the extension (his CPA I would presume) is who will have the paperwork proving it was filed.
Paid tax pros have a form 9325 that shows the extension was filed and accepted if they did it by computer.
