401(K) contributions

I contributed to a 401(K) plan from January 2021 through May 2021. Contributions were withdrawn directly from my paychecks into the 401. I left the job and withdrew the funds so I received a 1099-R form, which has been reported in my taxes. Do I also report the contributions that were withdrawn from my paychecks into the 401? Thanks!

401(K) contributions

The contribution made to the 401(k) account would already be reported on your W-2 in box 12 with a code of D.  You do not enter these contributions anywhere else on your tax return.

So I would not need to put anything under "Deductions & Credits">>"Traditional and Roth IRA Contributions"? I'm guessing this category is for individuals who have separate IRA accounts that are not through their employers?

Correct that is only for IRA contributions and not for tax deferred retirement accounts such as a 401(k).
