On box 12 i have a code D and a code DD so which one do i use

You enter both code D and code DD in separate lines for box 12 on your W-2. These codes refer to two different things.

Code D refers to the amount of 401(k) contributions that you made; code DD is the sum of all the money you and your employer contributed towards your health insurance premiums.

Confused about how to enter another box 12 line....look at the Software form again

There's a line just below box 12 (in the software) that says "+Add Another Row"
