You have to contact the issuer of the form for a replacement.
If it is from your bank account or investments you might be able to log into that account and download another copy of the 1099Div.
As mentioned by others, if you hold the shares in a brokerage account, contact your broker. But it sounds like that be the case. If you hold the shares directly, you might have luck contacting GE's transfer agent. See
