I am trying to fill a MA Withholding form for my new job. I need help in this regard.
I am around 28 years and my spouse is 35 years old with no children. what I have to write in Number 1 to 5, please.

Line 1 is “1”. You are under 65.
Line 2 is “4”. You are married to someone under age 65.
Line 3 is “0” You have no children (and I assume no other dependents, such as parents or other relatives that you are supporting)
Line 4 is “5”. Line 1 plus Line 2.
Line 5. Enter an amount if you want additional MA tax withheld from your pay. If you have a large amount of bank interest, stocks or other investment income, you may want to have an additional amount withheld to avoid owing at tax time.
Line A. Leave blank. You are filing married, not head of household.
No, that is not correct. The form is asking you to write '2 if you are 65 or older will be before next year. There is a period after the number 1, before he next sentence begins but it's difficult to see.
