This has been an issue for a number of Utah residents....several Utah people have noted that the had to go back to EVERY form that had UT listed in it's state line....W-2 forms, 1099-R forms, Unemployment 1099-G forms, etc, and triple check the exact Employer ID numbers they entered. in some cases, they missed one digit in the series of numbers, in other cases they found it entirely missing on their unemployment forms,
....(and in another they said using lower case "wth" as the final letters, instead of upper case worked but that's unconfirmed as to whether that was really the problem).
But this is only for Utah residents/workers/forms
