We file a joint return. Our income consists of my pension and my wife's social security. Your form suggests that we would have received a W-2 for her social security. We received a Form SSA-1099 but no W-2. The 1099 shows the deduction for Medicare premium. Your worksheet has a place for Medicare tax deduction. Is that the same? It would be useful if your worksheet was geared to the 1099 language.

If it's asking for an EIN number you are entering it in the wrong place. There is no EIN on social security SSA-1099.
Try going to the search box and type in SSA-1099 or Social Security Benefits. That will give you a Jump To link to take you directly to it.

Are you having trouble with where to enter your incomes?
Do not try to enter your SSA1099 or RR1099RB as a W-2. Go to Federal> Wages & Income>>Retirement Plans and Social Security (SSA1099 and 1099RRB) to enter your SSA1099.
When you enter your SSA1099 the Medicare that was paid from your SS will be entered and will flow automatically to medical expenses.
To enter your retirement income, Go to Federal> Wages and Income>Retirement Plans and Social Security>IRA 401 k) Pension Plan Withdrawals to enter your 1099R.
