I recently moved. I have entered my personal information to match my new address, but this does not match my 1099-SA which has my old address. I am to a form that is asking if my personal information matches my 1099-SA, and it of course does not, but I can't tell what I am supposed to do about it. Do I enter in the old address in this form, since it has the old address?
