If my 1099-SA address does not match my current address because I just moved, when it asks if these match, which address do I use?

I recently moved. I have entered my personal information to match my new address, but this does not match my 1099-SA which has my old address. I am to a form that is asking if my personal information matches my 1099-SA, and it of course does not, but I can't tell what I am supposed to do about it. Do I enter in the old address in this form, since it has the old address?

Yes, you can enter your old address on the 1099-SA because that is what it shows.  And don't worry, that address won't be used for anything on your tax return.  They just want it to match.  You probably should notify the issuer of your new address so next year's form will be correct and you won't get this error next year.
