Not sure what your question is. But whether or not you receive an information form (w2 or 1099-misc) doesn't matter. You still must report all income received from all sources unless there is a specific exclusion.
Check with uber but I believe they content that its drivers are not employees. Hence they do not receive W2s. Instead they received 1099-MISC forms and report that as a business income on Schedule C. If you made less than $600 uber would not be required to send you a 1099-MISC. But you are still required to report the income if you have to file otherwise (e.g. have other income) or if your self-employment income was more than $400. So in your case you have to report the $65 of uber income if you otherwise need to file.
