I received the NYS Healthcare Bonus - I need to fill out IT 225 to make sure NYS doesn't tax that amount, how do I do that?

I received the HWB that was taxed federally and should not be taxed by the state. I learned I needed IT 225 to make sure my AGI is accommodated for the amount I received so that I do not get more money taken out. But I dont see this form anywhere on TurboTax

In the New York interview, on the page for "Changes to Federal Income," under "Wage and retirement adjustments," click the Start button for "Health care and mental hygiene worker bonuses," then enter the amount of the bonus that was included in your W-2. TurboTax will make the necessary entry on Form IT-225 and exclude the bonus from your New York income.

But how do you properly fill that form out?

There are two boxes, federal & NYS Source Portion.

I know my bonus was $1000 and it was exempt from NYS taxation.

What do I put for both of those boxes?

If you are a NY state resident filing form IT-201 then only enter the bonus amount in Column A "Total Amount."  If you are a NY nonresident or part-year resident filing form IT-203 then enter the the total in Column A and the portion that relates to NY source income in Column B. 

 

If you use the step-by-step interview in TurboTax as @rjs mentions the amounts will populate the correct columns and forms for you.

Hi - I wanted to confirm there was nothing we needed to do on the federal side for this healthcare worker bonus ? I added it to the state under "healthcare worker and mental hygiene bonus" in state and I added in the same section where I added my paid family leave for federal.

That is correct. You would indicate it seperately from the paid family leave as a seperate line item on that Box 14

in the federal input. These two figures would all be included as part of your total Gross Wages in Box 1. As long as this

is the correct figure which you you reporting directly off the W2 that you received, then this is where it gets entered.
