it was new york life but i havent received a form to file my taxes as of yet. they were supposed to mail this by 01/31

Contact the disability insurance company and ask them what they are going to send to you and when they will take care of it. It is possible that you can go to their website for the document. Wait until you have everything to file your 2022 return---amending is a pain.
