I removed a child care provider but the address keeps showing up in my state form and it keeps asking me for details about the deleted provider. how do I fix this?

I had the same problem and found the answer here: https://ttlc.intuit.com/questions/2580507-how-do-i-delete-form-2441-in-turbotax-online

Essentially you have to re-do the steps in that section and remove your previous selections, otherwise TurboTax think you want that form included.
