Form 8962 is not correct

You didn't complete all the data required for the allocation.
Entries are required in Part IV of Form 8962, boxes (a) through (d), and at least one box (e) through (g), on the applicable lines 30 through 33, for amounts allocated for each shared policy.
The shared policy numbers you reported on Form 8962, Premium Tax Credit, Part IV, column (a), don't match the information we have on file from the Health Insurance Marketplace.
