I think you have some labeling errors in the 1099-R form, specifically, the "verify entry" page.
On the first page of 1099-R,
Box 12 is about FATCA
Box 13 is not there.
Box 14 is State Tax Withheld (first state)
Box 15 is the state and Number
Box 16 is State Distribution Amount.
All these labels and box numbers match up with my paper form.
Then I hit Continue, and see Let's Verify This Entry
This second form shows
Box 12 as Amount Withheld. The amount is correct for Amount Withheld, but the box number is wrong.
Box 13 is the State and Number.
Box 14 is State Distribution Amount. Matches State Distribution Amount in the first page, but wrong box number.
Boxes 15 and 16 are not shown on this page.
It looks like only the box numbers are wrong on this second page, because the text labels match up with the entries when comparing to the first page.
I'm not sure where else to report what looks like a software bug.

Yes, the TTX software is using the proper-official IRS 2020 1099-R forms with the NEW box numbering in the later numbers. For the prior year box numbers 12,13,14, they were re-numbered as 14,15,16, for 2020.
Problem is that a few organizations have failed to update their issued 1099-R forms for 2020, and that is confusing people ....(and appears to be what LBMD has experienced)

So far Iowa IPERS has admitted that the messed up.....
Ohio OPERS and one Insurance company (Allianz) appear to also have issued mis-formatted 2020 1099-R forms.

And yes Dan , there was a verification page that listed the boxes using the improper 2019 format...it's been reported and corrected in one of the latest software updates...so it's all boxes 14,15,16 now.

Thanks. That second page does not show up this time. It's possible that I typed something inconsistent the first time, or left out an entry that was expected. When that page did show up, it did seem like the box numbering was wrong, but it doesn't matter to me now. Thanks!
I'm having the exact same problem. The box numbers in TurboTax are not the same as on my 1099R. State income tax withheld has always been line 12 on the 1099R and TurboTax desktop has it as line 14 this year. I've deleted the entire entry and entered it again with the same result. I'm choosing to ingnore the error in TurboTax and enter the numbers where approriate, but TurboTax needs to correct their error.
The page in which I saw the error was a page for verification, not the initial data entry screen. I saw that verification page because I had done something wrong on the original data entry screen: I left out number in the state distribution box, 16, and that apparently was flagged by their algorithm. So they had me go over the numbers in the second screen. I'm not suggesting you made the same error I did, but maybe there's something inconsistent in your 1099R. They do seem to have an error in their form, but what I saw was an error in the verification page which shows up only if there's something amiss in the primary data entry screen.
I also had the same problem with the verifcation page and reentering the data seemed to clear that up. However the inital data entry page is in error. It asks for the state taxes withheld from box 14 on the 1099R be entered. There is no box 14 on my 1099R. My state taxes withheld has been in box 12 for as long as I have been receiving 1099Rs and TurboTax has always correctly asked for the amount to be entered from box 12. They have a clear error on this page that should be corrected.

My paper 1099-R does have State tax withheld in box 14, and that is what the primary data entry screen asked for. Apparently the paper form has changed since 2019. Last year, the state tax withheld was in box 12 in my paper form. My 2019 1099-R form did have a box 14, "state distribution." I was wrong when I said there is no box 13 this year; it is blank for me, "date of payment."

Thanks for clearing this up for me. My 1099R is issued by the federal Office of Personnel Management and clearly they used the old format.
NO ! boxes 12, 13, 14 would be the proper format for an OPM Form...
The OPM forms are not 1099-R forms...they are CSF- or CSA-1099-R forms that are supposed to be laid out differently (at least apparently so this year).
IF you are seeing state boxes 14,15,16 in the software entries (during the interview) then you started entering those in the wrong form-type. You need to delete that 1099-R and start a new one, but select the CSA-one : where boxes 12, and 13 are used (14 might not be presented to you ...or maybe the CSF- one if it's an after-death annuity)

IF you are using the Desktop software ?
..............the CSF- or CSA-1099-R box numbers are correct during the interview. Do not look at the forms in Forms Mode...those "Worksheets" move the values to whatever boxes are appropriate for the software to transfer the data properly. Thus, the Forms Mode "Worksheet" may not display in exactly the same way as on your paper forms...but the interview presentation will...
Thank you. I see the problem now. I'm using the desktop deluxe version and I imported last years data. I am not in the forms version.. When I got to the 1099R section I just edited the data. Either I missed it or it didn't take me to the page on the type of 1099R. It was checked as a regular 1099R and not the CSA 1099R. When I checked the correct box, I got the correct sequence of box numbers.

Thank you, Steam Train! I ran into the same problem with Home and Business, and thought my TT was defective. In fact, it was my State's Comptroller's Office, which, as you pointed out, did not update the box numbers to the 2020 version. As a result, my State tax withheld never got entered during the interview process (because 2020 has no Box 12 for it), and I nearly lost out on a fat State tax refund! I owe you one. Stay safe, my friend.
Connecticut also used the 2019 box numbering on its 1099-R pension forms.
