Do I file as self employed if I'm not and have a W-2 from my employer?

If you received a W-2 form you can not file as self-employed. However, if you were, in fact, self-employed and received payment in form of cash you can file as self-employed using Schedule C.

How do I report income from self-employment?

Under "Employment Expenses", I entered "Job-Related Expenses". I continue to get the message "Needs review". How do I get an actual dollar value in that field?
