I have a tex-flex account is that considered a HSA, SA or MSA

You should contact your employer to be certain, but you most likely have a flexible spending account (FSA) instead of a HSA, SA, or MSA.  If it is a flexible spending account for medical expenses, you do not need to report it on your tax return. 

If it were a Health Savings Account (HSA) that you and/or your employer contributed to then there would be an amount reported in box 12 of your W-2 with a Code W.
