OK ... since it makes absolutely NO difference as that info is simply immaterial in all ways ... just enter YOUR city/state/zip for your personal residence on the 1099-R form and move on.
Double check the zip code entered. If you imported the w2, some odd data may have gotten stuck in the transfer causing the issue. You may need to delete the form and re-enter it manually.
However, Turbo tax did not carry that over as it has for the last 9 years and I had to type it in again (3X now) with exactly the same 1099-R ( address, TIP, and info as it has been the last 9 years). Turbo tax changes it to unknown country and OC currency after I attempt to correct it. 4 hours ,so far is way, way more than the 45 min. estimated time to complete like all the other years and I didn't even have look up property taxes, etc. with the new standard deduction.
