I received a 1099-SA form from the original holder of my HSA, but my employer switched banks for our HSAs in September of 2014. From the second bank, I received a 5498-SA form. I filed the 1099-SA in the return. How do I handle the second form (5498-SA) in my federal return?

Form 5498-SA is a statement of contributions. In your case of employment contribution, the amount shown on 5498-SA should have already been on your W2 so there's no need to enter it anywhere else. The 5498-SA is for your information only; you don't actually enter it directly.

DURING THE END CHECK, YOUR SOFTWARE REQUIRED AN ENTRY FOR FMV HSA AND IT CHANGED MY REFUND!!!!! NOW I CANNOT FIND WHERE YOU WANTED IT SO I CAN CHANGE IT

They may not match if you had after tax contributions in addition to pretax employer contribution.
"Why would they be different?"
F5498-SA can include more than pretax employer contribution.

I made an extra contribution to my HSA and it was reported on the 5498 however when I try to add it to turbotax it lowers my refund instead of increases it! This is as of 3/18/15 so the latest fix isnt working for this particular scenario.

@bmccarthy the refund indicator is not a reliable measure. You must examine the actual tax forms to determine how your HSA contribution is treated.

So I have $1,750 on my 5498-SA and my W2 in Box 12a has a C for 174 and DD for 2665.08... I'd like to get credit for the $1,750, but Turbo Tax is not letting me take it anywhere.. help!

Then your employer has reduced your W2 wages by the $1750 but did not report code W $1750 in box 12. You can enter it in the HSA section. Look for the line "Employer Contribution not Reported on a W2".

I don't think the employer reduced my W2 wages lines, 1, 3,5 and 16 all match. Any ideas? Thanks for the help..

If you are certain of no reduction in reported wages, then it's an after tax contribution and it would be entered on the line "All Other Contributions". This will be a tax deduction on F1040, line 25.
