Where is the federal ID and subscriber number on the1095-c form

This Form 1095C is for information only and is not required to be entered on the tax return.

Answer the Health Insurance questions and if you have or expect to receive a Form 1095A because of marketplace insurance wait until you receive that to complete your return.

I did get my 1095 A and now I see the employer started paying me health insurance in May but

I didn't stop my healthcare,
