What is the Ins. Co subscriber no? It is asking me to enter it but I already checked off the box where it says I never got the 1099-HC form.

What is the Ins. Co subscriber no? It is asking me to enter it but I already checked off the box where it says I never got the 1099-HC form.

Your insurance card would also have this number on it. If you have lost it you can just call the ins. co. and they can easily provide it to you.
