What do I do if I have not received a 1099-INT form for my HSA account at American Fidelity? California wants my HSA earnings, and I don't have them.

Option 1: Call American Fidelity Customer Support and request your 1099-INT form.

Option 2:  Log in to your American Fidelity account and check for HSA tax forms, including the 1099-INT.

Option 3: Manually calculate your HSA earnings by reviewing your monthly statements. Add up ‌interest, dividends, and any other earnings for the year.
