When you entered your W-2 form, the amount in box 10 for dependent care benefits is taxable to you until you account for it, by either indicating who you paid the funds to or if you didn't use them, you need to indicate that.
After you enter the W-2 form, you will be asked if your job provided child care on-site, and then if you had a flexible spending account. If you answer "yes" that you had an FSA, you will be asked what the leftover amount was in the account. You need to enter the amount in box 10 on your W-2 for that amount so your benefits won't be taxable.
Once you do this you should not be asked about the form 3506.
