I received a 1099R for rollover of funds that were transferred from one custodial account to another. Box 2a has $0.00 in it. box 2b has Total distribution checked. Box 7 has a G. Should I request a corrected 1099R from the custodian that distributed the funds since they were never sent directly to me? Should I request a form from the custodian receiving the funds verifying their deposit in a qualified account?

No, In May you will be issued form 5498. Just make sure you show as a rollover on your taxes. It odd but the IRS will match your 5498 with your 1099-R after the April 15th deadline.
