All state return pages not printing?

I efiled my federal taxes and printed my AZ state taxes to send by mail. The pages on the state return are numbered; however, it stops at page 4 of 6.  When I look at the pdf version, it is the same.  I don't know for certain if I'm missing something needed.  I am guessing probably not, but I don't have a page 5 or 6.

Pages 1 and 2 are the calculations and the final number which are the most relevant. The remaining pages are schedules describing amounts on certain lines in the first 2 pages.  For example line 36 on the Arizona 140 reverts to page 6. I am assuming that line 36 is blank, which is why page 6 is omitted.
