$300 penalty for private use mail

How do I do the $300 penalty for private use form I got ? I was told to use a 1040v and make that into a money order to collect the $300. Do I put that on a 1099 or do I send that im separated? And no , I am not penalized. It’s a offering from the IRS.

I don’t know what you are talking about.   Did you get a letter from the IRS to pay something?  The IRS only sends letters, they don’t call or email you.  I would call the IRS and see what they want or it could be a scam.
