Sorry, but you don't provide enough information to answer that....could be any number of things and all anyone reading this is left with, is guessing.

One couple possible things,   

 

1) make sure you are entering a CSA-1099-R and not a 1099-R.

2)  ....during the  Federal error check step, there may be a screen that shows the state income and withholding lines for boxes 14, 15, 16....dn you need to check a box to confirm the numbers are correct.

 

(But read the labelling for each box...not sure the box numbers match your CSA-1099-R....the subject is important)

Is it asking about box 5 insurance?   That appears to be a bug that has been reported.

However I have not been able to reproduce it.
