Box 16 is for the amount of your distribution attributed to the state. On a 1099-R, generally box 16 is the same as the amount in box 1. 

Do you have anything in boxes 14 and 15? If box 14 is empty (this reports any tax withholding paid to the state), you do not need to enter anything for box 16. If box 14 and 15 are filled, enter the amount from box 1 for box 16 also.
