Well... that's problematic as the code in box 7 determines tax treatment of your distribution. I recommend that you contact the administrator of the form and ask for the corrected form, which should display the code. You can't determine the code, but the administrator must...

Otherwise, you would have to prepare substitute form 1099-R, where you enter your own code and and attach an explanation as to why it is substitute.
