RMD I did not receive a 1099 how do I enter

I have taken the complete 2018 RMD out of one account but did not receive a 1099 do I fill out the 1099 info as I best know?

The custodian of the IRA that you took the distribution from is required to send you a Form 1099-R showing the distribution. Call them and ask for it. Make sure they have your correct address.
