If I took a loan less than the max allowed against my 401k do I need to claim it in taxes?

And what form would I expect to get to show me what needs to be entered?

A loan from a 401(k) is not reported on a tax return.

If you took a loan secured by your 401(k) and paid it normally, there is nothing to report on your tax return.
