My previous employer provided me with a retirement plan, basically an annuity, which pays me X dollars every month. How do I answer the question about RMD?

Just "yes", it is RMD and "All" is RMD and you took all your RMD (providing you did if you have other retirement accounts) and you are finished.
This is the correct way to do this.
