1099-r was issued with no info in box 7. amount $147.04. How do i file return with info in Box 7

Received 1099-R from court appointed administrator.(previous employer in settlement with class Action law suit).1099-R received with any  Distribution code in Box 7. 

Gross Distribution was only $147.04.

A code is *required* in box 7 on a 1099-R.
Are you sure that the code is not a "7" and you are mistaking the code for the box number?

I note your attachment and removed it because it contained personal information on a public website.

Contact the 1099-R issuer for a corrected 1099-R.   The 1099-R is not valid without a code in box 7.
