Box 10a should list the state, in your case IA
Box 10b is the state ID, in your case 42-6004574 (you might need to not enter the hyphen)
Box 11 is whatever state taxes were taken out. That I cannot tell you, you need check the form.
You can use the link below to get a copy of your 1099-G, maybe the one you have got cut off.
Same issue as the other guy, no 10a or 10b or 11, obviously the state is Iowa so that answers 10a and there is a box with the state taxes withheld amount though it’s not numbers as box 11 but rather labeled as just “Iowa income tax withheld” but that still leaves the question of 10b. The number you gave is the federal tax ID for IDR (state equivalent of IRS) not the state ID for unemployment which is what box 10b is supposed to be
There are 2 ways to input your 1099G for unemployment. Please delete the copy you have entered. Instead, go to the new section titled Unemployment. It should be under the W2 section when you choose what you want to work on.
Entering the form there will not ask you about all the extra boxes.
@AmyC The poster is saying that Iowa is not using the IRS approved 1099-G form but is using Iowa's own version of the form that actual does have state tax withheld but it is not in box 11 (there is no box 11). Also the Iowa form does not include the payers state ID number that TurboTax requires.
FYI Maryland is doing the same thing. They report Federal tax withheld in box 3 and state tax withheld in box 4. There are not boxed 10a, 10b and 11 at all on the form.
I have reported this to the forum moderators to pass on to the developers to find out what is going on but so far no response. Perhaps you can pass it on.
I don’t have a Iowa state payer number it won’t let me go on without it, and it is not on the form
The Iowa Department of Revenue's ID Number is 42-6004574 for purposes of entering a Form 1099-G.

It is displaying Iowas FEDERAL ID number, but not the STATE ID number.
