My question has to do with the fact that my payment was made to a physical therapy clinic and not to the individual that handled my therapy. I have a similar question regarding my co-pay for doctor's visits. The co-pay is made out to a medical clinic and not to the individual doctor. Would this still be listed under Fees to Medical Professionals.
